If you have not completed your online Pre-Check registration and your medical history form prior to your appointment, we recommend arriving 15-20 minutes early to complete your forms.
Medical Record Requests
Hillsboro Eye Clinic respects the health information rights for our patients and has partnered with Provider1st to securely provide medical records and billing copies to patients, guardian/legal representatives or 3rd party requestors with HIPAA authorization.

If you are a patient and would like your medical records instantaneously, please log into the patient portal and download your records.
